HPCSA Registration for Foreign Qualified Medical Specialists Where to Begin

The process involves more than submitting your degree. Your qualifications, professional registration and specialist training must be considered, and the requirements depend on the registration category you are applying for.

Start by confirming your registration pathway
Before gathering documents, confirm which HPCSA registration category applies to your circumstances. This helps you identify the correct application forms, supporting documents and assessment requirements.
Where required for your pathway, you will also need an endorsement letter from South Africa’s National Department of Health. This endorsement and HPCSA registration are separate steps, so confirm the requirements for each.
Prepare your supporting documents
A useful starting checklist includes:
- Your initial medical degree, with copies notarised as required.
- Official academic transcripts, with sworn English translations where necessary.
- A valid Certificate of Good Standing from the relevant medical regulatory authority.
- A passport copy certified or notarised in the format required for your application.
- Evidence of your medical registration.
- Internship duty certificates.
- Your specialist qualification and specialist registration
- Detailed information about your specialist training, where requested.
A professional Certificate of Good Standing should come from the relevant medical regulatory authority. A university letter does not replace this document.
Document formats, certification requirements and validity periods should be checked against the current HPCSA checklist before submission. BLCSA can assist with arranging notarisation when you have your original documents available.
Complete your credential verification
Internationally qualified applicants must have the required credentials verified through ECFMG’s Electronic Portfolio of International Credentials (EPIC).
For specialist applicants, these include the medical diploma, academic transcripts, internship certificate, medical registration certificates, specialist qualification and specialist registration certificate.
The current process begins with establishing a MyIntealth account and an EPIC Portfolio. Applicants must arrange for the required verification reports to be sent to the HPCSA. ECFMG states that the HPCSA will not consider the application until all required credentials have been verified and the reports received.

Understand the specialist assessment process
Once your documentation and verification requirements are complete, your application can proceed through the relevant HPCSA assessment pathway.
Specialist qualifications and training may require further evaluation, including curriculum review, peer review or assessment involving the Colleges of Medicine of South Africa (CMSA), as directed by the HPCSA. Additional examinations or requirements may apply.
Submitting an application does not automatically lead to registration. The HPCSA makes the registration decision based on the applicable requirements and assessment outcomes.
Allow time for the process
Credential verification and specialist assessment can take several months. The timing depends on responses from issuing institutions, the completeness of your application and any further assessment required.
Avoid treating six months as a guaranteed maximum. Confirm the expected processing time for your individual application before making firm employment or relocation commitments.
